Leonard Joseph Dvorak, 83, of Toledo, passed away Tuesday, November 19, 2019 at the Iowa Lutheran Hospital-Unity Point in Des Moines. A Memorial Visitation will be held Friday, November 22, 2019 from 5-7 PM at Kruse-Phillips Funeral Home in Tama. Private Interment will be held at Stayskal Cemetery in rural Vining, Iowa at a later date.

A memorial fund will be established.
